
Family Support -what is it?
PLANS - People Leading Accessible Networks of Support -- is a Family Support 360 program which supports people with developmental disabilities to live and work in the community of their choice. Our mission is to empower people to achieve their dreams through choice of services and supports.
Who is eligible?
Covered services
A PLANS Coordinator assists each person to develop a plan to access a broad range of supports and services within the local community.
PLANS offers a limited pool of funds to be used for supports and services chosen and directed by each person.
Supports and Services include, but are not limited to the following:
· Service Coordination-Community Resources and Services
· Companion Care
-Recreation and Leisure Opportunities
-Community Living Skills Training and Practice
-Independent Living Skills Training and Practice
· Personal Care Services and Supplies
· Adaptive Equipment
· Respite Care
· Employment Services
Referral process
Referrals can be made directly to the PLANS Coordinator at the Career Learning Center (394-5120) or through the Resource Coordinator for the Division of Developmental Disabilities at the One-Stop Career Center (394-2302). These Coordinators work with all interested individuals and their families to determine eligibility, assisting them to explore all options available in order to seek the most appropriate program from which to receive supports.
